TEACHER ACKNOWLEDGEMENT FORM

We are delighted to extend an offer of admission to your child at Crossmaze – Preschool & Day Care. Before finalizing the admission process, we kindly request you to carefully read and acknowledge the terms and conditions outlined below. This acknowledgement is an essential step to ensure a clear understanding between the school and the parents/guardians.
 

  1. Admission Acceptance: I/We, the undersigned, acknowledge the offer of admission for my/our child, at Crossmaze – Preschool & Day Care.

  2. Financial Responsibilities: I/We understand and accept the financial terms, including tuition fees, extracurricular activities, and any other related charges as specified by the school.

  3. Code of Conduct: I/We have familiarized ourselves with, understood, and agree to adhere to the school’s code of conduct, rules, and regulations. We commit to maintaining respectful and professional behavior at all times.

  4. Participation in School Activities: I/We support and encourage my/our child’s active participation in school activities, events, and programs.

  5. Communication: I/We agree to maintain open and regular communication with the school regarding my/our child’s academic progress, behavior, and any other relevant concerns.

  6. Emergency Contact Information: I/We will promptly inform the school of any changes to our contact details, including emergency contact information.

  7. Health and Medical Information: I/We confirm that all health and medical information provided is accurate and up to date. I/We authorize the school to seek emergency medical treatment for my/our child if required.

  8. Withdrawal Policy: I/We understand the school’s withdrawal policy and agree to follow the specified procedures in the event of withdrawing my/our child from Crossmaze – Preschool & Day Care, and will clear all outstanding dues.

  9. Parental Involvement: I/We recognize the importance of parental involvement in my/our child’s education and commit to actively participating in school-related activities.

  10. Admission Age Criteria:  As per Karnataka Government guidelines, the minimum age for Grade 01 admission is 6 years as on 1st June of the academic year (subject to applicable notifications). Parents are requested to select the appropriate program (Play Group, Nursery, Junior KG, Senior KG) based on the child’s age. Any deviation from the prescribed age criteria shall be entirely at the parents’ discretion and responsibility, and the school will not be liable for any consequences arising from such deviation.
